Systematic Desensitisation and Exposure Hierarchies
Systematic desensitisation, developed by Joseph Wolpe in the 1950s, is one of the foundational behavioural techniques for treating phobias. It is based on the principle of reciprocal inhibition — the idea that it is physiologically impossible to be simultaneously relaxed and anxious. By pairing gradual exposure to the feared stimulus with deep relaxation, the anxiety response is progressively weakened and replaced with a calm, manageable response. Whilst modern approaches to phobia treatment have evolved beyond strict Wolpean desensitisation, the principles of graduated exposure and anxiety hierarchy construction remain central to evidence-based practice.
The exposure hierarchy is a personalised, ordered list of situations related to your phobia, ranked from least to most anxiety-provoking. For a spider phobia, the hierarchy might begin with looking at a cartoon drawing of a spider (anxiety rating perhaps 10/100), progress through looking at photographs, watching a video, being in a room where a spider is present in a container, and eventually handling a live spider (anxiety rating 100/100). For a driving phobia, the hierarchy might begin with sitting in a parked car, then starting the engine, then driving in a car park, then driving on a quiet residential street, and eventually driving on a motorway.
The hierarchy is constructed collaboratively with your therapist and should contain at least ten to twelve items, with relatively even spacing between anxiety ratings. Too large a jump between steps will make the next exposure feel overwhelming; too small a jump will make progress unnecessarily slow. Each step is practised repeatedly until the anxiety it provokes has diminished substantially (typically to 50 per cent or less of the initial rating) before moving to the next step.
Modern exposure-based treatment also incorporates inhibitory learning principles, which emphasise that the goal of exposure is not simply to reduce anxiety (habituation) but to create new learning that competes with the original fear association. This is achieved by maximising the violation of expectancy — ensuring that what happens during the exposure is meaningfully different from what the individual predicted. This approach produces more robust and generalisable treatment gains.
