Specific Phobias: When Fear Becomes Disproportionate

A specific phobia is an intense, irrational fear of a particular object or situation that is out of proportion to the actual danger posed. Common phobias include fear of spiders (arachnophobia), heights (acrophobia), enclosed spaces (claustrophobia), blood and needles (blood-injection-injury phobia), flying (aerophobia), dogs (cynophobia), and vomiting (emetophobia). Whilst many people have mild aversions to these things, a specific phobia is diagnosed when the fear causes significant distress or impairment in daily functioning.

Phobias are maintained primarily through avoidance. By consistently avoiding the feared object or situation, you never have the opportunity to learn that it is not as dangerous as your anxiety suggests, that you can cope with the discomfort, or that your anxiety will naturally subside if you remain in the situation. Avoidance also tends to generalise over time, spreading from the specific trigger to related situations and progressively restricting your life.

The most effective treatment for specific phobias is graded exposure therapy, which involves creating a fear hierarchy and systematically working through it from the least to the most anxiety-provoking steps. Treatment is often remarkably brief — many specific phobias can be resolved in as few as one to five sessions of focused exposure work. The key to successful exposure is remaining in the feared situation until your anxiety has noticeably decreased, rather than escaping at the point of peak anxiety.

Blood-injection-injury phobia is unique among phobias in that it is associated with a vasovagal fainting response (a drop in blood pressure and heart rate) rather than the typical fight-or-flight response. Treatment for this subtype includes applied tension, a technique in which you repeatedly tense the large muscles of your body to raise blood pressure and prevent fainting during exposure to blood or needles.
