Interoceptive Exposure: Facing Your Fear of Physical Sensations

Interoceptive exposure is a specialised technique used in the treatment of panic disorder that involves deliberately inducing the physical sensations you fear in a safe, controlled environment. The purpose is to demonstrate that these sensations are not dangerous and to break the automatic association between bodily sensations and catastrophic interpretations that drives the panic cycle.

Common interoceptive exposure exercises include breathing through a narrow straw for two minutes (to induce breathlessness), spinning in a chair for thirty seconds (to induce dizziness), running on the spot for one minute (to induce a racing heart and sweating), hyperventilating for sixty seconds (to induce tingling, light-headedness, and a feeling of unreality), and tensing all muscles for one minute then releasing (to experience muscle tension and release). Each exercise is designed to produce the specific sensations that trigger your panic response.

The exercises are repeated multiple times until the anxiety response to the sensation diminishes significantly. You then practise the exercises in increasingly challenging contexts — at home alone, in public places, and in situations where you have previously experienced panic attacks. Throughout, you are learning that the sensations themselves are harmless, that they pass naturally, and that you do not need to escape or perform safety behaviours to survive them.

Interoceptive exposure is typically introduced after psychoeducation about the fight-or-flight response and cognitive restructuring of catastrophic misinterpretations. When combined with these components, it forms a highly effective treatment package with recovery rates exceeding seventy-five per cent.
