Harm Reduction: Practical Strategies for Safer Substance Use

Harm reduction is a pragmatic, evidence-based approach to substance misuse that focuses on minimising the negative consequences of substance use rather than requiring complete abstinence as a precondition for support. Originating in public health and now widely endorsed by UK services, harm reduction recognises that not everyone is ready or willing to stop using substances immediately, and that even modest reductions in use or improvements in the safety of use can produce significant health benefits.

Practical harm reduction strategies for alcohol include setting a drink limit before you start drinking and sticking to it, alternating alcoholic drinks with water, avoiding drinking on an empty stomach, keeping track of units consumed (the UK Chief Medical Officers recommend no more than 14 units per week, spread over three or more days), and planning drink-free days each week. For other substances, harm reduction strategies include using in the company of trusted people, starting with a small dose to test strength and purity, avoiding mixing substances (which significantly increases risk), using clean equipment, and knowing the signs of overdose and how to access emergency help.

Harm reduction is not in conflict with the goal of eventual abstinence or significant reduction — rather, it meets people where they are and provides a pathway towards lower-risk use and, for many, eventual cessation. Research consistently demonstrates that harm reduction approaches engage individuals who would otherwise not access services, and that engagement with harm reduction services is associated with subsequent reductions in use and improved health outcomes.
