Exposure and Response Prevention (ERP): A Practical Guide

Exposure and Response Prevention is the gold-standard psychological treatment for OCD, with decades of research demonstrating its effectiveness across all subtypes of the condition. ERP involves two complementary components: exposure (deliberately and systematically confronting the situations, objects, or thoughts that trigger your obsessions) and response prevention (refraining from performing the compulsions that you would normally use to reduce the resulting anxiety).

The process begins with creating an exposure hierarchy — a list of triggers ranked from least to most anxiety-provoking. You then work through the hierarchy gradually, starting with items that provoke moderate anxiety (typically rated 40-50 on a 0-100 scale). During each exposure, you deliberately remain in contact with the trigger without performing any compulsions, rituals, or safety behaviours, and you rate your anxiety at regular intervals. With sustained exposure, anxiety naturally decreases through a process called habituation.

For contamination OCD, exposures might involve touching "contaminated" surfaces and not washing. For checking OCD, leaving the house and not returning to check. For harm OCD, holding a knife whilst near a loved one and not performing mental rituals. For symmetry OCD, deliberately leaving things uneven or asymmetrical. Each exposure is designed to provide new learning: that the feared outcome does not occur, that the anxiety is tolerable and temporary, and that you can function effectively even when anxious.

ERP is challenging — it requires you to voluntarily experience anxiety rather than avoiding it. However, the discomfort is temporary and purposeful, and most people begin to notice significant improvements within the first few weeks of consistent practice. Your therapist will guide you through the process at a pace that is challenging but manageable, and will work with you to address any obstacles that arise.
