Body Image and Self-Worth: Separating Identity from Appearance

Body image disturbance is a central feature of eating disorders, but difficulties with body image affect a far wider population. Body image is not simply about how your body looks objectively — it is a complex psychological construct that encompasses how you perceive, think about, feel about, and behave towards your body. Negative body image involves a distorted perception of your body's shape and size, persistent negative thoughts about your appearance, feelings of shame or disgust when you think about or look at your body, and avoidance behaviours (such as refusing to look in mirrors, wearing baggy clothing, or avoiding situations where your body might be seen).

In cognitive behavioural terms, body image difficulties are maintained by a set of core beliefs about the importance of appearance — for example, "My worth as a person depends on how I look," "If I am not thin, I am not acceptable," or "People judge me primarily on my appearance." These beliefs drive a cycle of body checking (weighing, mirror-checking, comparing yourself to others), body avoidance, and compensatory behaviours (dieting, excessive exercise, cosmetic procedures) that reinforce negative body image rather than improving it.

Therapeutic work on body image involves challenging the overvaluation of shape and weight in determining self-worth, reducing body checking and avoidance behaviours, developing a broader basis for self-evaluation (relationships, achievements, values, character), and learning to treat your body with compassion and respect regardless of its current shape or size.

It is worth noting that body image difficulties are significantly influenced by cultural and media messages about idealised body types. Developing media literacy — the ability to critically evaluate the images and messages you are exposed to — is an important protective factor.
